  • blood group mismatch (ABO incompatibility)
  • anaphylaxis
  • hypercalcaemia (citrate binds to Ca++)
  • hyperkalaemia (transfusion of loads of old blood)
  • preservatives (leading to acidosis)
  • hyperthermia (?)
  • Hb can poison kidneys
  • coagulopathy
  • fluid overload
